Population Health Improvement
Goal: Reduce mortality attributable to cardiovascular disease in GM and save over 500 lives by 2018
a) We aim to increase the uptake of NHS Health Checks in the MAHSC partners to over 70% within 2 years.
b) We aim to have an integrated screening and prevention system developed, tested and implemented across MAHSC partners within 5 years.
c) We are aware that in years 1 to 3 the focus of our CVD portfolio is on those at risk of disease and the delivery of services. In addition, our domain has responsibility to work with MAHSC partners to identify upstream determinants of health such as lifestyle (smoking, obesity), genetics, and social factors (worklessness, housing) which could be addressed as a MAHSC priority in year 2 and beyond. To this end, we will integrate the work of Public Health England and Professor Aidan Halligan into Domain plans to ensure we have a unified view on the single priority by April 2014 and a joint delivery plan by July 2014. We will be engaged in the design of a large scale improvement programme to address social determinants of health and commence delivery of this programme no later than 2015.
